“Is breast augmentation really safe?” – Many of my patients ask me this question at the beginning of our first consultation. The clear answer is yes – if you entrust yourself to an experienced specialist.

I myself have never experienced a serious complication during a breast augmentation in my entire career. And that is precisely why I would like to show you transparently in this article what the theoretical risks are – and why these are generally very low in a qualified specialist practice.

Straight to the point – How high is the risk really?

Breast augmentation is one of the most frequently performed plastic and aesthetic procedures in the world – and is also very safe. According to the German Society of Plastic, Reconstructive and Aesthetic Surgeons (DGPRÄC), serious complications only occur in around 1-3% of cases. At around 5-10%, capsular fibrosis is the most common complication, but in most cases it is mild and does not require treatment.

In all the years that I have been performing breast augmentations, I have not experienced a single serious complication. Why have I never had any serious cases? It’s down to careful planning, the right method, a gentle surgical procedure and intensive aftercare.

What risks can theoretically occur?

As safe as breast surgery is – as with any medical procedure, there are theoretically possible complications. It is important to be aware of these possible complications and to assess them realistically. Here is an overview of the most important ones:

  • Infections

Thanks to modern hygiene standards and careful surgical techniques, infections are very rare nowadays. If an infection does occur, it usually manifests itself as redness, pain, swelling or fever. In most cases, this can be treated quickly and effectively with antibiotics. In very rare cases, if the infection is more serious, it may be necessary to temporarily remove the implant to allow the surrounding tissue to heal completely. The implant can then be reinserted.

  • Capsular fibrosis

The formation of a capsule of connective tissue around the implant is a normal reaction of the body and is not usually a problem. However, some patients may experience excessive hardening of the capsule, which is known as capsular fibrosis. This can lead to feelings of tightness, hardening or even visible deformation of the breast. Lighter forms of capsular fibrosis are harmless in most cases and do not require treatment. In more severe cases, a second operation may be necessary to remove the capsule or replace the implant. Thanks to modern implant technologies and gentle surgical techniques, this risk has been further reduced in recent years.

  • Implant rupture or displacement

Thanks to the high quality of modern implants, cracks in the implant material (ruptures) are extremely rare nowadays. Even if a rupture occurs, the silicone retains its shape thanks to the cohesive properties of the material and does not cause any damage to health. Displacement of the implant, for example due to physical activity or the healing process, can occur in individual cases. Both cases can be corrected surgically if necessary.

  • Scarring

During breast surgery, the incisions are made in such a way that the scars are as inconspicuous as possible, for example in the natural fold of skin under the breast. The visibility of the scars depends on individual wound healing, the surgeon’s technique and aftercare. In most cases, the scars fade after a few months and become almost invisible. Proper care and avoiding direct sunlight during the healing process make a decisive contribution to this.

  • Emotional changes

After the operation, slight sensory disturbances in the breast or nipples are not uncommon. These can be perceived as numbness, tingling or increased sensitivity. The reason for this is that fine nerve branches can be affected during the operation. In most cases, the sensation returns to normal within a few weeks to months as the nerves regenerate.

  • Bleeding or hematomas

Bruising (hematomas) is a common but generally harmless side effect after surgery. They usually occur shortly after the procedure and disappear on their own within a few weeks. Severe bleeding in which blood accumulates in the surgical area (so-called hematoma) is rare. However, should this occur, a short-term intervention is usually necessary.

  • Asymmetry

Perfectly symmetrical breasts are just as rare with natural breasts as they are after breast surgery. Slight differences in the size, shape or position of the implants are completely normal and are hardly noticeable in most cases. In very rare cases, however, there may be noticeable asymmetries that can be corrected with minor corrective surgery.

  • Anesthesia risks

As with any procedure under general anesthesia, there are theoretical risks ranging from circulatory problems to allergic reactions. However, such complications are extremely rare as the anesthesia is administered by experienced anesthetists.

  • Very rare: BIA-ALCL (breast implant-associated anaplastic large cell lymphoma)

BIA-ALCL is an extremely rare form of cancer that in rare cases is associated with certain implants, particularly textured implants. However, the risk of this is extremely low and is less than 0.03%. In Germany, strict regulations apply to the approval of implants, so that only high-quality and tested products are used. Thanks to modern implants and careful aftercare, this risk is now virtually eliminated.

What influences the risk?

Not every risk affects every patient in the same way. The following factors influence the likelihood of complications:

  • Patient’s state of health: Previous illnesses, skin condition or nicotine consumption can influence wound healing.
  • Type of implant: High-quality, CE-certified implants significantly reduce the risk of complications.
  • Surgical access: Access routes such as the underbust crease are considered particularly safe.
  • Surgeon’s experience: The surgeon’s routine and specialization are crucial for an optimal result.

How can risks be minimized?

You can play a big part in making your breast surgery as safe as possible – especially by choosing an experienced specialist who has extensive experience in breast surgery. Choosing the right surgeon is one of the most crucial steps in minimizing potential risks and achieving an optimal result.

In addition to choosing a plastic and aesthetic surgeon who specializes in breast surgery, these factors play a decisive role:

  • Comprehensive advice and realistic clarification of expectations
  • Complete medical information and careful preliminary examination
  • No smoking for at least 2 weeks before and after the operation
  • Observe all aftercare instructions (e.g. wear a support bra, take it easy)
  • Regular check-ups and, if necessary, imaging procedures for follow-up care

Uncertainties are human – and this is precisely where regular aftercare creates confidence and security. Regular check-ups are particularly important to monitor the healing process and ensure long-term safety. Examinations by your specialist can detect any abnormalities at an early stage. Additional imaging procedures such as ultrasound or MRI provide additional security and enable a precise assessment of the implants and the surrounding tissue. These follow-up appointments not only give you clarity, but also the good feeling that you are being well looked after so that you can continue to feel comfortable.

Fast recovery method – gentle and safe

In my practice, we have developed the Fast Recovery Method for breast surgery ourselves. This specially designed technique enables us to reduce tissue trauma to a minimum – resulting in a particularly fast recovery and an exceptionally low complication rate. You benefit from:

  • short surgery duration (approx. 45 minutes)
  • Minimal bleeding, less pain medication required
  • less pain, many patients are already mobile the next day
  • faster healing with a high level of safety

In the many years of my surgical career, I have performed countless breast augmentations – with consistently positive experiences.

My conviction: If all the parameters are right – information, technique, aftercare and trust – the risk is extremely low. There has never been a serious complication in my entire career. That’s why I attach great importance to transparent communication, honest advice and individual care. This is the only way to create the trust that is crucial for a successful procedure.

Conclusion – breast augmentation is safe if the general conditions are right

Yes, there are risks – as with any medical procedure. But modern plastic surgery is so advanced today that serious complications have become the exception. In professional hands, with high-quality materials, gentle methods and good aftercare, breast augmentation is a safe and proven procedure.
