Definition of

What is gynecomastia?

Gynecomastia refers to abnormally enlarged breasts in men. A distinction is made between

  • true gynecomastia – enlargement of the mammary gland tissue, usually not influenced by exercise or diet.
  • Pseudo-gynecomastia – accumulation of fat in the breast region, often caused by obesity and treatable with exercise or weight loss.

In many cases, gynecomastia occurs on both sides, but it is also possible for problems to occur unilaterally. It is important to remember that gynecomastia is not the disease, but merely a symptom. Depending on the degree of severity, a feeling of pressure, tightness and pain are possible and increase in proportion to the size of the breast. However, pain can also be completely absent. Those affected often suffer psychologically from enlarged male breasts.

However, gynecomastia does not always have to be pathological and permanent. Although this is often the case in adult men, gynecomastia can occur in adolescents as a side effect of puberty. As a rule, it then subsides with further development, so that those affected have normal-sized breasts towards the end of puberty.

Symptoms of gynecomastia

There are three different degrees of gynecomastia:

Grade 1: The breast is slightly enlarged, as are the mammary glands. There is still no excess skin and the gynecomastia is not yet perceived as particularly disturbing.

Grade 2: A slight excess of skin can be seen, the breast is already significantly enlarged, causes some problems with movement and can cause slight pain. An underbust fold is not yet visible.

Grade 3: The breast is greatly enlarged and resembles the female breast shape. Both a clear excess of skin and an underbust fold are visible. Movement is increasingly restricted and pain occurs frequently.

Treatment methods

Breast reduction options for men

For true gynecomastia

If the glandular tissue is enlarged, it can be surgically removed. The procedure is performed through fine incisions in the skin and – in the hands of experienced specialists – is considered particularly safe.

For pseudo-gynecomastia

If the enlarged breast is due to excess fatty tissue, gentle liposuction is used. This method leaves only minimal scars and enables a natural result with a short recovery time.

Combination of both methods

A so-called mixed finding often occurs – i.e. both increased fat and glandular tissue. In such cases, we combine both methods to achieve a flat, masculine breast contour.

Many years of expertise and the highest safety and quality standards allow us to perform gynecomastia surgery in our Frankfurt practice on an outpatient basis so that patients can return home on the same day. We use state-of-the-art methods and technologies, such as VASER® liposuction, a procedure that not only removes excess fatty tissue but also uses ultrasound to precisely define the contours of the pectoral muscle. This method is particularly suitable for athletic patients who want an aesthetically defined and masculine chest.

How does gynecomastia surgery work?

Dr. Abuagela bei Beratung

The preparation

Blood-thinning medication should be discontinued approximately two weeks before the breast reduction – in consultation with your doctor. Nicotine and alcohol should also be completely avoided during this time. On the day of the procedure, we ask you to come to our practice sober. This means: please do not consume any food or liquids for at least six hours before the operation. During a personal consultation on the day before or morning of the operation, we will discuss all the steps again in peace and quiet and will be available to answer any questions you may have – so that you feel well prepared and confident. Together we will choose the best option for your individual situation – always with an incision that is as minimal and inconspicuous as possible.

The treatment

The operation begins with small incisions that are as inconspicuous as possible to minimize scarring. We make these incisions either directly through the nipple or at the edge of the areola – depending on your individual anatomy. Excess mammary gland tissue is first removed through these minimal incisions. VASER® liposuction is then performed through the incisions already made – or, if necessary, through additional incisions placed laterally in the armpit. This technique not only gently removes excess fatty tissue around the pectoral muscle, but also emphasizes the natural definition of the muscle.

The aftercare

The first few days after the procedure are usually uncomplicated, but may be accompanied by slight pain – depending on the individual’s perception of pain. Our patients usually report a well-tolerated healing process, which we specifically support with suitable painkillers. To promote healing and optimize the treatment result, we recommend wearing special compression garments for several weeks. You should refrain from sport and physically strenuous activities for around 4 to 6 weeks. Light activities and a return to everyday working life are usually possible after 3 to 7 days, depending on your individual well-being.

And most importantly: you are not alone with your questions or worries. We are always at your side, even after the operation.

FAQ

Answers to frequently asked patient questions about gynecomastia surgery

What helps with gynecomastia?

The cause of gynecomastia should first be clarified. It is usually harmless – but disturbing. Gynecomastia can occur as a result of puberty, for example, but this is not permanent. If it is caused by obesity or genetic predisposition, liposuction can help.

Is the treatment of gynecomastia covered by health insurance?

As a rule, this is a purely aesthetic procedure that is not covered by health insurance.

How long does pubertal gynecomastia last?

In most cases, gynecomastia disappears in affected adolescents after a few years. However, it should be clarified whether pathological changes are present.

Can gynecomastia return after surgery?

Yes, this is possible. As hormonal reasons are usually responsible for enlarged male breasts, this should be examined in detail.

What can cause gynecomastia?

Gynecomastia can be caused by hormonal changes, genetic predisposition, obesity or certain medications. It often occurs in three phases of life: in newborns (due to maternal hormones), during puberty (temporary breast growth) and in old age (due to decreasing testosterone and increased fatty tissue). Diseases of the liver, kidneys or hormone-producing organs as well as tumors can also cause abnormal gynecomastia. A thorough medical examination should therefore always be carried out before treatment.
